A 2018 meta-analysis of 7 randomized trials and 627 participants found that exercise training reliably changes the craniovertebral angle, the standard measurement of forward head posture. The same analysis found the effect on pain was weaker and less consistent. Posture is measurable and modifiable. What it predicts is a separate question.
Does forward head posture actually cause neck pain?
The evidence is genuinely mixed, and that matters before anyone spends six weeks on chin tucks. A 2024 systematic review in Coluna/Columna pooled 4 low-risk-of-bias observational studies covering 1,067 adults and found no association between “text neck” and the onset of neck pain. In a randomized trial of 30 female office workers with chronic neck and shoulder pain, 2 minutes per day of high-intensity elastic band lateral raises over 10 weeks was associated with a 40% reduction in pain intensity and a 6% increase in isometric strength compared to controls. The sample was small, and the finding needs replication at scale.
